Remove double chin
When we meet other people, the first thing they look at is our eyes. Pronounced bags and dark circles under the eyes can easily give the impression that we are tired or exhausted, perhaps even that we have stayed awake at night or hardly slept at all, although this is not the case. Sometimes they also make the face look falsely sad or older than we actually are.
Understandably, many sufferers decide to have their lower eyelids tightened and the annoying bags under their eyes permanently removed. This minor, low-pain procedure is one of the most frequently requested operations at our specialist practice in Frankfurt and is ideal for visibly rejuvenating your appearance, opening up your eyes and restoring their youthful freshness. Arrange a non-binding consultation with Dr. Nichlos now!
A lower eyelid lift (medically known as a lower eyelid blepharoplasty) is used to remove excess skin and fat in the lower eyelid area, which is responsible for dark circles or bags under the eyes that are perceived as aesthetically unpleasant. The aim is to make the face look younger, fresher and more vital again.
The main reason for the sagging of the tissue is the loss of elasticity and resilience of the skin due to the ageing process. External influences such as stress, an unhealthy lifestyle, lack of sleep or a genetic predisposition can also promote the development of dark circles and bags under the eyes.

Dr. Emmanouil Nichlos has more than 17 years of experience in lower eyelid lifts and aesthetic eyelid surgery.
With a keen understanding of the delicate eye area and modern treatment techniques, he achieves natural, harmonious results. During the recovery phase, patients who have recently undergone surgery receive reliable care and can contact Dr. Nichlos personally at any time, even around the clock, if necessary.
Lower eyelid surgery is suitable for patients who are bothered by bags under the eyes, dark circles, or sagging skin under the eyes and wish to achieve a more refreshed appearance. Dr. Nichlos is best able to determine whether this procedure is the right solution for you during a personal consultation. Experience has shown that good results can be achieved in the following cases, among others:
For dark circles under the eyes and tear troughs
Despite getting regular rest and enough sleep, you have dark circles under your eyes and always look tired. Concealer can only mask this superficially. In most cases, this is caused by age-related changes in tissue volume, which make the transition between the lower eyelid and the cheek appear very harsh. During a lower eyelid lift, Dr. Nichlos redistributes the tissue, making this area appear brighter and more harmonious again.
Over the years, the skin on your lower eyelids has sagged and now forms unsightly folds. These not only make your eyes appear smaller, but also give you a sad look, even though you’re not feeling that way at all. During a lower eyelid lift, Dr. Nichlos removes the excess skin. This immediately makes your eyes look larger and more refreshed, and the mistaken impression of sadness disappears.
As we age, the muscles and connective tissue of the lower eyelid can become so lax that the weight of the excess skin causes the lower eyelid to turn outward. In medical terms, this is known as senile ectropion. The problem is that the conjunctiva is exposed, often causing the eyes to water and dry out at the same time. Frequent inflammation is also a typical symptom.
Without treatment, chronic symptoms can develop, potentially leading to corneal damage. With a lower eyelid lift, Dr. Nichlos can restore the eyelid to its natural position and relieve you of these symptoms. As a bonus, your eyes will look younger and more alert again.
In modern plastic and aesthetic medicine, there are two methods for lower eyelid lifts:
In this procedure, the specialist makes a very fine incision along the lash line. Through this incision, they can remove excess skin and protruding fatty tissue as needed, tighten the muscle if necessary, and even reposition fat downward. The latter helps smooth the transition to the cheek in cases of a pronounced tear trough. Once healed, scars are barely visible.
In transconjunctival blepharoplasty, Dr. Nichlos makes the incision not on the outside but on the inside of the lower eyelid. This has the advantage of leaving no external scars and fully preserving the natural tension of the eyelid. The method is well-suited for younger patients with under-eye bags who do not, however, have significant excess skin.
Which method is ideal for you depends on your individual situation and your preferences. Please feel free to schedule a no-obligation consultation with Dr. Nichlos.
Yes, in many cases this actually makes sense. That’s because it would be wrong to consider the lower eyelids in isolation during treatment. To achieve a harmonious result, it’s important to also address areas such as the forehead, eyebrows, cheeks, and upper eyelids, since these areas influence one another. The following procedures are useful additions to a lower eyelid lift:
The most common procedure at our practice in Frankfurt. Dr. Nichlos performs upper and lower eyelid lifts in a single session, ensuring a harmonious overall result.

Drooping eyebrows further accentuate the appearance of tired eyes. During an eyebrow lift, Dr. Nichlos opens up the eyes by gently lifting the brows.

Sometimes the problem isn’t too much tissue, but too little. During the same procedure, Dr. Nichlos can inject the patient’s own fat into the tear trough or cheeks to create a natural transition. So-called “nanofat” is particularly effective for this purpose. This specially processed form of autologous fat is rich in stem cells and growth factors.
Unlike normal autologous fat, nanofat is not primarily aimed at adding volume, but rather at regeneration. It thickens the already extremely thin skin under the eyes, stimulates natural collagen production, and is thus able to visibly reduce dark discoloration. This effect cannot be achieved with fillers or a simple tightening procedure alone.
Surgery isn’t always necessary to further enhance the results of a lower eyelid lift. Minimally invasive procedures such as Botox and/or hyaluronic acid are also effective options. Dr. Nichlos uses these, among other things, to restore volume under the eyes or at the temples, to create a more harmonious transition from the lower eyelid to the cheek, or to smooth crow’s feet and fine expression lines around the eyes.
Dr. Nichlos would be happy to discuss with you in person which treatment or combination makes the most sense for you.
If you are bothered by bags under your eyes and/or dark circles and would like a permanent solution with a lower eyelid lift, we invite you to a no-obligation consultation. In a relaxed atmosphere, after a brief examination of your eye area, Dr. Nichlos will explain to you exactly how he removes the annoying bags under your eyes, which methods he uses and how the operation can change your appearance for the better.
Each lift is individually planned and precisely tailored to the shape of your face and eyes so that the result looks very harmonious and natural. So you don’t have to worry about your characteristic facial expression being altered by the operation.
At the end of the preliminary consultation, Dr. Nichlos will inform you about possible risks and how you should prepare for the appointment. This includes discontinuing blood-thinning medication (2 weeks before the operation, in consultation with your doctor) and avoiding alcohol and nicotine to prevent wound healing problems.
On the day of your operation, you will come to us at the Berger Clinic. Here, Dr. Nichlos and his team will welcome you in a state-of-the-art operating room equipped with all the technical refinements of plastic and aesthetic medicine to ensure the highest possible quality of treatment.
You should allow 1.5 to 2 hours for the lower eyelid lift. As a rule, Dr. Nichlos performs eyelid surgery under local anaesthetic, which is very gentle for you and does not require a longer stay. As soon as the area under the eyes has been anaesthetized and Dr. Nichlos has marked the bags under the eyes or excess skin to be removed, he makes a short incision along the eyelid contour with a scalpel (external approach). Alternatively, an approach from the inside via the conjunctiva is also possible (transconjunctival approach), which has the advantage of leaving hardly any visible scars.
In pure skin tightening, excess skin is first removed and then the remaining tissue is tightened. When correcting bags under the eyes, protruding fatty tissue is also reduced or redistributed.
The plastic and aesthetic surgeon then sutures the surgical wounds with fine stitches and closes the treated area with a plaster bandage. You can then leave the clinic immediately. However, we recommend that you do not drive yourself, but bring someone with you, as swelling caused by the operation may restrict your field of vision.
You should allow yourself plenty of rest in the first few days and sleep with your upper body elevated if possible. Please take care not to touch or rub the area around your eyes. Initially, the area below the eyelashes will be swollen as a result of the operation and bruising may occur. You can effectively counteract this by cooling the area.
You can return to work just a few days after lower eyelid surgery. You will be fully socially acceptable after about 10 days. You should refrain from sports and other strenuous activities for at least 2 weeks after lower eyelid surgery. The same applies to the application of make-up, as the fine particles can easily penetrate the wounds and cause irritation.
Approximately 5 to 7 days after the treatment, you will come to our practice for suture removal and the first follow-up check. Here, Dr. Nichlos will take a look at the healing process and give you tips on how you can optimally care for the area in the coming months and thus support the healing process.
The removal of bags under the eyes and excess skin on the lower eyelid is one of the most frequently performed procedures in plastic and aesthetic surgery and is associated with low risks when performed professionally by an experienced specialist such as Dr. Nichlos.
As with any surgery, post-operative bleeding, infections or wound healing disorders can occur during lower eyelid skin tightening. However, by adhering to strict hygiene and quality standards, the risk of these complications can be reduced to a minimum.
What cannot be prevented due to the nature of the treatment is temporary swelling and bruising under the eyes. In some cases, sensitivity disorders, slight dryness of the eyes or increased lacrimation may also occur. However, side effects of this kind usually subside on their own within a few days to weeks.
Many patients want to know the costs associated with a lower eyelid lift before undergoing treatment. Since every eye area is unique, the exact price may vary depending on the findings and the extent of the procedure.
At our practice in Frankfurt, the cost of a lower eyelid lift typically starts at:
The exact costs depend, among other things, on how pronounced the under-eye bags are and whether excess fatty tissue needs to be removed or redistributed. Following a personal consultation, Dr. Nichlos will create a customized treatment and cost plan that transparently lists all services.
The total cost of a lower eyelid lift consists of several components. These typically include:
In addition, there are anesthesia costs and clinic fees, as the procedure is performed in a specialized clinic.
To help you plan your treatment flexibly, you also have the option of financing the costs of a lower eyelid lift in convenient monthly installments.
Financing is provided by an experienced financing partner and can be tailored to your individual financial situation. This means you don’t have to pay the full cost upfront but can settle the treatment through monthly payments.
We would be happy to inform you about the requirements and the financing process during a personal consultation.
A lower eyelid lift is useful if bags under the eyes, excess skin or wrinkles in the lower eye area are perceived as bothersome – regardless of age. In many cases, patients opt for the procedure from the age of 40. However, correction may also be advisable at an earlier age, depending on the extent of the problem.
No, the lower eyelid lift is a very painless procedure. You may only feel the injection of the local anesthetic for a few seconds. During the operation, you may notice a feeling of pressure or a slight pulling sensation. After the procedure, the effect of the local anesthetic will last for about 2 to 3 hours. If pain occurs afterwards, you can relieve it with mild painkillers such as paracetamol.
Yes, even pronounced bags and dark circles under the eyes can be removed with a lower eyelid lift, making the eye area look fresher, younger and healthier again.
Scars cannot always be completely avoided during lower eyelid surgery. However, Dr. Nichlos can reduce the visibility by making a precise incision directly on the lash line. Or, if possible, he chooses the transconjunctival approach from the inside via the conjunctiva, which leaves no visible scars.
With good scar care, the lower eyelid correction scars will continue to fade over the following months so that they are barely visible at some point. During your consultation, Dr. Nichlos will explain to you what you need to consider when caring for your scars in order to achieve the best possible result.
In most cases, local anesthesia is sufficient for a lower eyelid lift. For more complex findings or on request, the procedure can also be performed under twilight sleep or general anesthesia. The individually suitable form of anesthesia is determined together in a personal consultation.
Swelling and hematomas may occur on the lower eyelid after the operation. However, experience shows that these will subside within 4 to 5 days, so that you will already have a first impression of the eyelid lift. However, it will take around two to three months for the scars to heal completely and the final swelling to subside. By then at the latest, you will be able to see the final result of your lower eyelid correction in the mirror.
The result of a lower eyelid lift in Frankfurt is permanent, as the excess skin and fatty tissue under the eyes is completely removed. However, as the skin ages, the skin around the eyes may become loose again, causing bags under the eyes to reappear. If desired, these can be removed with a further lower eyelid blepharoplasty. You can help to maintain the result yourself by protecting your eyes from UV radiation, eating a healthy diet, avoiding smoking and keeping your skin well moisturized.
As a rule, you will be socially acceptable again after around 7 to 10 days. Slight swelling or bruising may be visible at first, but this usually subsides quickly. Dr. Emmanouil Nichlos will give you individual advice on when you will be able to take part in social life again without restrictions.
As with any surgical procedure, there are certain risks associated with a lower eyelid lift. These include temporary swelling, hematomas or slight asymmetries. In rare cases, there may be a feeling of dryness, irritation or wound healing disorders. Dr. Emmanouil Nichlos will inform you in detail about possible complications during your personal consultation.
Yes, even very well. In our practice, we combine a lower eyelid lift with an upper eyelid lift or facelift to achieve an all-round harmonious result. A combination with autologous fat transfer and nanofat (stem cell) treatment is also possible. Feel free to talk to Dr. Nichlos about the possibilities during your consultation.
After a lower eyelid lift, you should refrain from wearing contact lenses for a few days so as not to impair the healing of the eye area. Glasses can usually be worn again more quickly as long as they do not exert any direct pressure on the treated area. Your specialist will advise you individually on this.
