What are the different methods of lower eyelid lift surgery?
In modern plastic and aesthetic medicine, there are two methods for lower eyelid lifts:
Incision at the edge of the eyelid (transcutaneous)
In this procedure, the specialist makes a very fine incision along the lash line. Through this incision, they can remove excess skin and protruding fatty tissue as needed, tighten the muscle if necessary, and even reposition fat downward. The latter helps smooth the transition to the cheek in cases of a pronounced tear trough. Once healed, scars are barely visible.
Access via the conjunctiva (transconjunctival)
In transconjunctival blepharoplasty, Dr. Nichlos makes the incision not on the outside but on the inside of the lower eyelid. This has the advantage of leaving no external scars and fully preserving the natural tension of the eyelid. Yes, in many cases this actually makes sense. That’s because it would be wrong to consider the lower eyelids in isolation during treatment. To achieve a harmonious result, it’s important to also address areas such as the forehead, eyebrows, cheeks, and upper eyelids, since these areas influence one another. Autologous fat transfer
Sometimes the problem isn’t too much tissue, but too little. During the same procedure, Dr. Nichlos can inject the patient’s own fat into the tear trough or cheeks to create a natural transition. So-called “nanofat” is particularly effective for this purpose. This specially processed form of autologous fat is rich in stem cells and growth factors.
Unlike normal autologous fat, nanofat is not primarily aimed at adding volume, but rather at regeneration. It thickens the already extremely thin skin under the eyes, stimulates natural collagen production, and is thus able to visibly reduce dark discoloration. This effect cannot be achieved with fillers or a simple tightening procedure alone.
Botox and hyaluronic acid
Surgery isn’t always necessary to further enhance the results of a lower eyelid lift. Minimally invasive procedures such as Botox and/or hyaluronic acid are also effective options. Dr. Nichlos uses these, among other things, to restore volume under the eyes or at the temples, to create a more harmonious transition from the lower eyelid to the cheek, or to smooth crow’s feet and fine expression lines around the eyes.

On the day of your operation, you will come to us at the Berger Clinic. Here, Dr. Nichlos and his team will welcome you in a state-of-the-art operating room equipped with all the technical refinements of plastic and aesthetic medicine to ensure the highest possible quality of treatment.
You should allow 1.5 to 2 hours for the lower eyelid lift. As a rule, Dr. Nichlos performs eyelid surgery under local anaesthetic, which is very gentle for you and does not require a longer stay. As soon as the area under the eyes has been anaesthetized and Dr. Nichlos has marked the bags under the eyes or excess skin to be removed, he makes a short incision along the eyelid contour with a scalpel (external approach). Alternatively, an approach from the inside via the conjunctiva is also possible (transconjunctival approach), which has the advantage of leaving hardly any visible scars.
In pure skin tightening, excess skin is first removed and then the remaining tissue is tightened. When correcting bags under the eyes, protruding fatty tissue is also reduced or redistributed.
The plastic and aesthetic surgeon then sutures the surgical wounds with fine stitches and closes the treated area with a plaster bandage. You can then leave the clinic immediately. However, we recommend that you do not drive yourself, but bring someone with you, as swelling caused by the operation may restrict your field of vision.
What happens after the lower eyelid lift in Frankfurt?
You should allow yourself plenty of rest in the first few days and sleep with your upper body elevated if possible. Please take care not to touch or rub the area around your eyes. Initially, the area below the eyelashes will be swollen as a result of the operation and bruising may occur. You can effectively counteract this by cooling the area.
You can return to work just a few days after lower eyelid surgery. You will be fully socially acceptable after about 10 days. You should refrain from sports and other strenuous activities for at least 2 weeks after lower eyelid surgery. The same applies to the application of make-up, as the fine particles can easily penetrate the wounds and cause irritation.
Approximately 5 to 7 days after the treatment, you will come to our practice for suture removal and the first follow-up check. Here, Dr. Nichlos will take a look at the healing process and give you tips on how you can optimally care for the area in the coming months and thus support the healing process.
